Understanding MCV (Mean Corpuscular Volume): What Your Red Blood Cell Size Tells Us About Your Health
In my practice, I often see patients who come in after a routine blood test showing an “abnormal MCV.” Many are unsure what this actually means, especially when they feel otherwise fine. MCV, or Mean Corpuscular Volume, is a simple but very important indicator that tells us the average size of your red blood cells. And surprisingly, this small number can give us big clues about your overall health.
What is MCV?
MCV is part of a Full Blood Count (FBC) test. It measures the average size of your red blood cells. Red blood cells are responsible for carrying oxygen throughout your body, so their size and quality matter.
- Normal MCV: usually 80–100 fL
- Low MCV: smaller-than-normal red blood cells
- High MCV: larger-than-normal red blood cells
What Does Low MCV Mean?
When I see a low MCV, the most common cause is iron deficiency anemia. Patients often report tiredness, pale skin, dizziness, or shortness of breath.
Other causes may include:
- Chronic blood loss (e.g., heavy menstruation or gastrointestinal bleeding)
- Thalassemia (a genetic blood condition, relatively common in our region)
- Long-term poor iron intake
Low MCV usually tells us the body is producing smaller red blood cells, often due to lack of hemoglobin production.
What Does High MCV Mean?
A high MCV suggests larger red blood cells than normal. In clinic, I often associate this with:
- Vitamin B12 deficiency
- Folate deficiency
- Liver disease
- Excess alcohol intake
- Certain medications
Patients may experience fatigue, numbness in hands or feet, or memory issues in more advanced cases.
Why MCV Alone Is Not Enough
One important point I always explain is that MCV cannot be interpreted alone. It must be read together with:
- Hemoglobin level
- Red blood cell count
- Iron studies
- Vitamin B12/folate levels
A “normal MCV” does not always mean everything is fine, and an “abnormal MCV” does not always mean severe disease.
When Should You See a Doctor?
You should seek medical evaluation if you have:
- Persistent fatigue
- Dizziness or fainting
- Unexplained pale appearance
- Abnormal blood test results (MCV, hemoglobin, or RBC changes)
