Knee pain is one of the most common problems we see in clinic. Many patients come in after hearing about different types of knee injections — steroid injection, hyaluronic acid injection, PRP injection, or even stem cell-related treatments.

Among these, PRP injection has become increasingly popular because it sounds natural, modern and “regenerative”. Some patients are told that PRP can repair the knee, regrow cartilage, or delay surgery. Understandably, this makes PRP very attractive.

However, as doctors, we need to look at knee pain more carefully.

PRP may have a role in selected patients, but it is not suitable for everyone. More importantly, PRP should not be chosen simply because it sounds advanced. The correct treatment for knee pain should always begin with a proper knee assessment.

What Is PRP Injection?

PRP stands for Platelet-Rich Plasma.

 

It is prepared from the patient’s own blood. A small amount of blood is taken and processed to concentrate the platelets. The platelet-rich plasma is then injected into the painful area, such as the knee joint.

 

Platelets contain growth factors and biological signals that may support tissue healing and reduce inflammation in certain conditions. This is why PRP is often described as a biological or regenerative treatment.

 

But it is important to understand one thing clearly:

 

PRP is not a guaranteed cartilage-regrowth treatment.

 

For knee osteoarthritis, the effect of PRP may vary depending on the patient, the severity of the knee condition, the preparation method, the injection technique and the overall treatment plan.

Why Do Some Patients Consider PRP for Knee Pain?

Many patients are attracted to PRP because it uses their own blood. Compared with steroid injection, PRP may sound safer or more natural. Some patients also prefer PRP because they want to avoid repeated painkiller use or delay knee replacement surgery.

 

In selected patients with early or moderate knee osteoarthritis, PRP may help reduce pain and improve function for a period of time. Some studies have reported improvement in knee pain and movement after PRP injection.

 

This is why PRP is not something we should simply dismiss.

 

However, the key question is not only whether PRP can help.

 

The more important question is:

Is PRP suitable for this patient’s knee condition?

The Potential Benefits of PRP Injection

PRP may have several potential advantages in selected cases.

First, PRP is prepared from the patient’s own blood. This means the risk of allergy is generally low.

Second, PRP may help some patients with mild-to-moderate knee osteoarthritis, especially when the knee is not yet severely damaged.

Third, PRP may be considered by patients who are not ready for surgery and want to explore non-surgical options.

The Limitations of PRP Injection

The biggest issue with PRP is that it is not one standard product.

PRP from one clinic may not be the same as PRP from another clinic. The concentration of platelets, the presence of white blood cells, the preparation machine, the activation method, the number of injections and the injection protocol may all be different.

This means PRP treatment is difficult to compare.

A patient may ask, “Doctor, is PRP good?”

But the more accurate question should be:

What type of PRP? Prepared how? Injected where? For which knee condition? At what stage of arthritis? With what follow-up plan?

Without these details, it is difficult to promise results.

Another limitation is that PRP may not work well for severe knee osteoarthritis. If the knee joint is already badly worn, if the joint space is severely narrowed, or if the knee is already “bone-on-bone”, PRP may offer limited benefit.

PRP can also be more expensive than other injection options. Some patients may need more than one injection, and the total cost can become significant.

Patients should also understand that PRP usually does not give immediate relief. Unlike steroid injection, which may reduce inflammation quickly, PRP may take weeks or months before any improvement is felt.

PRP Is Not Always the First Treatment I Recommend

Personally, I do not recommend PRP as a routine first-line injection for every knee pain patient.

This does not mean PRP is useless. It means PRP should be selected carefully.

In real clinical practice, knee pain can come from many causes, not only cartilage wear. A patient may have knee osteoarthritis, but the pain may also be contributed by muscle weakness, poor walking mechanics, meniscus problems, tendon inflammation, gout, excess weight, joint swelling, or overloading during daily activity.

If we inject PRP without understanding the real cause of pain, the result may be disappointing.

This is why I usually advise patients not to rush into PRP just because it sounds natural or regenerative.

The knee must be assessed first.

What Should Be Assessed Before Knee Injection?

Before deciding on any knee injection, we should understand several things.

  • How long has the knee been painful?
  • Is the pain worse when walking, climbing stairs, squatting or standing up?
  • Is there swelling?
  • Is there stiffness in the morning?
  • Is the knee unstable?
  • Is the pain from the joint itself, or from surrounding soft tissue?
  • Is there gout or inflammation?
  • How severe is the osteoarthritis?
  • Is the muscle around the knee strong enough to support the joint?
  • Does the patient need physiotherapy before or after injection?

These questions matter because knee injection is not just about choosing a medicine.

It is about choosing the right treatment for the right knee at the right stage.

Why Injection Technique Matters

Even when an injection is suitable, technique still matters.

The knee joint is a structure with specific spaces and surrounding tissues. If the injection is not placed accurately into the intended area, the result may not be optimal.

This is why ultrasound-guided injection can be helpful. It allows the doctor to see the knee structure and guide the injection more accurately.

For knee pain treatment, the experience of the treating doctor, the assessment process, the injection technique, and the post-injection plan are all important.

A good injection is not only about what is injected.

It is also about why, where, when and how it is injected.

PRP vs HA Injection vs Steroid Injection

Many patients compare PRP, hyaluronic acid injection and steroid injection.

Steroid injection may give faster relief, especially when the knee is inflamed or swollen. However, it may not be ideal as a repeated long-term solution without proper assessment.

PRP is a biological option, but the evidence and preparation methods are variable. It may help selected patients, but it is not a guaranteed solution and may not be suitable for severe arthritis.

Hyaluronic acid injection is commonly used for knee osteoarthritis to support lubrication and smoother joint movement. However, the result still depends on patient selection, injection accuracy, knee condition and rehabilitation planning.

Therefore, the question should not be, “Which injection is the best?”

The better question is:
Which injection is suitable for my knee condition?
